Highly Infectious, Not Very Lethal

Still focused on the Diamond Princess, because it’s a microcosm of the new coronavirus outbreak, isolated from all the others. We should get a fairly good idea of just how infectious it is, and just how lethal. The passengers and crew of this ship have become the guinea pigs in a medical experiment. And there seem to be all different races present.

A headline I read today –

The news from the cruise ship moored in quarantine in Japan is awful – coronavirus cases aboard are soaring

– probably arose from the fact that there were first 10 cases, and then 20 cases, and now 60 cases. But this may simply be because the results from some 270 or so tests have been only slowly coming out over a period of several days. It looks like it took 3 days for 270 tests to be completed, so they could only do about 90 tests per day.

So what it probably means is that when the tests were carried out, nearly a quarter of the passengers sampled were already infected.

And that shouldn’t be too surprising. As far as I can see, life aboard these sorts of cruises is pretty much one long party. People go on them primarily in order to socialize. So they’ll all have been mingling with each other closely during the 15-day voyage. Timeline:

The checks began after an 80-year-old Hong Kong man who had been on the ship last month fell ill with the virus.

He boarded the cruise ship in Yokohama on 20 January and disembarked in Hong Kong on 25 January.

Where had he been before he boarded in Yokohama? If he’d just arrived from Wuhan, he was probably a carrier. If not, then he likely picked it up on board from some other carrier.

The captain announced the discovery of coronavirus on 3 Feb. Two days later, 5 Feb, passengers were confined to their cabins. Same day, 10 cases were found on the ship. Next day, 6 Feb, another 10 were found. Today another 40. But these numbers are probably just the results of 270 tests carried out (on 4 Feb?) slowly trickling out.

If they get round to testing everybody, it rather looks like they’ll find that 60/270 or 22% of everyone on board  is carrying the virus.  And that will be something approaching 1000 cases.

However, 19 days after the 80-year-old Hong Kong man boarded the ship, nobody has died. And if the virus had been on the ship for 10 days before he arrived, then in an entire month nobody will have died.

So, using just the Diamond Princess numbers, it looks to me like this is a highly infectious virus, but not a very lethal one.

Some people think that the virus is racially targeted, and only Chinese people can contract it. But since a Brit who doesn’t look in the least bit Chinese has tested positive, this would seem not to be true.

Other people think that the new coronavirus epidemic is just another scare. Or that lung problems and pneumonia in Wuhan are being blamed on the virus, but actually caused by air pollution. Wuhan maybe just happens to be the worst place anyone would want to be when they come down with flu.

My guess, for what it’s worth, is that this new coronavirus is being massively hyped, because the dangers of absolutely everything – from tobacco smoke to carbon dioxide – are vastly exaggerated these days. So this latest threat is also being inflated into something worse than it actually is.

    People on a cruise already have an advantage of the privileged, with better living conditions, food medical attention etc before they even boarded. They are not ordinary people. I think the Diamond Princess is a fascinating experiment. Keep us updated with your thoughts….

      Good point. They’ve got plenty of good food. And they’ve got plenty of fresh air too, from my understanding of the ship’s heating and ventilation. And they’re under close medical supervision, The same probably isn’t true in Wuhan, for a lot of people.

    And smokers, although they are not allowed to smoke in their cabins or on balconies, if they have one.
    I don’t expect to see any deaths as the 2019-nCoV positive patients have been ferried to hospitals.

    In the meantime, the BBC announced that the WHO is trying to wring a few hundred million dollar out of us all right now.

      Germany now has a 14th case.(Just to update the Hopkins site quickly, since it appeared to take them a long time accept the second, third etc. cases) – the 13th case took them 1 full day(!) before it appeared on their site – the same it took them to acknowledge the third UK case (British citizen, Caucasian) who travelled to Singapore.
      Again,that is why, what I would like to call Second-hand transmission, confirming why authorities, Germany has 12 of it’s now 14 cases in BAVARIA (Karma is a bitch) due to 1 business meeting (The 2 others came back on the first flight to bring citizen back from China). By now the Bavarians (total smoking ban since 2010, just to mention since we will be blessed with a forthcoming paper that puts smokers in the position of being more likely to catch the disease, thus spreading it in the incubation stage) have been trained to be compliant citizen. (No smoking in front of children etc etc)
      Well, that didn’t prevent the children currently being made comfortable in Bavaria. I don’t expect ANY of them to be in any danger. We have had plenty of time to prepare without facing any shortages in face masks or diagnostic kits. They are not cheap over here let alone China. No hospital will carry any more kits for corona virus than absolutely necessary. (or dictated)

    As many have said, it’s not just the virus, it’s our reaction to it. The story here is a chap sprays alcohol all over himself as a disinfectant, gets into his car, lights a cigarette, then torches himself.

    There’s a grisly image of said gentleman, so you’re warned. Seemingly they’re doing the same in their apartments with similar results. Videos about 60 seconds.

    There is quite a lot to catch up with this morning. The BBC just announced there are 5 British people (one is a child) infected in France. Interestingly, in the report the BBC still does not accept the person-to-person transmission during the incubation stage.
    Germany has 3 children infected. It is one family and this case is well documented. The mother was the last one to be diagnosed.

    Meanwhile, three more cases of nCoV have been confirmed aboard the ‘Diamond Princess’, the Carnival-owned cruise ship currently under quarantine in Yokohama. Three more individuals have tested positive, bringing the number aboard to 64, the government said Saturday. All passengers are facing a two-week quarantine.

    I have stopped using the WHO web site for the reason that it lags at least 1 day behind. I am sorry but I am also not satisfied with the information the site provides.
    I do collect data made available for ONLY (!!!!) personal use as this is a novel virus and simply caught my interest. Also, it would be nice to discover once the epidemic is over that the numbers provided match up and that we could look confidently to Organisations should there be another novel virus epidemic somewhere sometime.

    Am I panicking? Absolutely not. I am not getting onto “runaway train” theories, either. For me it is a simple and effective step to just ignore them and only rarely will I challenge any of them.

    I view The WHO’s steps with concern as it here provides another runaway train:

    Chief medical officer for England Professor Chris Whitty said this morning that a further patient has tested positive for coronavirus in England.

    It comes after a Brit in Majorca was diagnosed with the virus, while five tested positive in France.

    Prof Whitty said the latest infection was linked to France. “The new case is a known contact of a previously confirmed UK case, and the virus was passed on in France,” he said.

    Here is where things are getting a little weird. Spain is listed as having 2 cases. Neither of them is Spanish, though.
    The first case determined was on the small island of LaGomera, a German tourist. The second case is (I am unsure where the patient is right now) on the island of Majorca, an English tourist.

    On the diamond princess 69 people were diagnosed with the coronavirus.
    The nationalities of the newly diagnosed passengers have not been revealed to Princess Cruises representatives, public relations director Negin Kamali told USA TODAY late Saturday.

    The cruise ship is one of several affected by the deadly coronavirus outbreak, which has prompted cruise lines to issue quarantines and perform testings and screenings. Meanwhile, some ports are denying cruise ships entry for fear of the virus spreading.
    Perhaps all cruise ships out there need to head home? Don’t cruise ships rely on their stops along their routes for picking up supplies?

    It looks like the last evacuation flight from Wuhan arrived home today.
    The passengers also included British government staff and military medics and arrived at RAF Brize Norton in Oxfordshire on Sunday morning.

    Two British nationals were not allowed to board the flight after they initially failed a temperature checks, although later checks were normal.

    One of them, Josh Skarratts, told Sky News: “Now we are stranded in Wuhan having missed the last plane out of Wuhan and our last means of evacuation.”
    For all that it is worth, I do hope the remaining Brits contact EVERY embassy in China. As far as I know there are still evacuation flights planned. All evacuees landing in any country will be quarantined for 2 weeks, so it does not matter where this is. Keeping fingers crossed for the lads.

    Sixty more cases of coronavirus have been confirmed on the Diamond princess cruise ship, domestic broadcaster TBS TV said via Twitter. NHK Japan is also saying around 60 new cases have been identified, quoting the health ministry.

    There are 130 cases on board now.

    Incubation time of 2019-nCoV
    = 2 – 14 days
    First transmission(s) by the 80 year old infected passenger occurred anywhere from 20.1. to 25.1.

    How many people was this man in contact with?

    The next question concerns the ships ventilation system. Can we safely exclude cabin to cabin cycling of air? It seemed a little odd that once passengers were confined to their cabins those who had booked a “smoker cabin” no longer were allowed to smoke in there. I am not sure if there wasn’t a balcony smoking ban prior).

    I do recall a video from a very frightened American woman who (I believe!) has been now diagnosed as being infected. Unfortunately I have not found any new videos from remaining passengers yesterday. I will search today again.

    May I politely appeal to the social media and Google to remove the WHO imposed (sorry, I do not know which other term is more appropriate) internet censorship which creates even more conspiracy theories.

    The UK has declared the Wuhan coronavirus a “serious and imminent threat to public health,” as cases were confirmed in more than 25 other countries and territories around the world.

    At this point in time it is unknown if 2019-nCoV also spreads airborne and the UK is taking (in my opinion) sensible precautions.

    I note this down (as I have Chris Martenson’s latest video) and continue with my life as per usual. Weekend is over and I have a lot of things to do today ranging from cleaning to shopping to meeting friends in a pub about half way from our houses. (The latter I am not happy with as it will be rather uncomfortable outside in the “high-ceiling” smoking “room”. But then, I don’t think I will stay long in the place).

    Perhaps there aren’t enough people at the John Hopkins site or the people working there are simply overwhelmed (?) – it’s no use to speculate, so: oh, whatever.
    The number of confirmed number of total cases on the John Hopkins site is now 64 cases lower than on the site I use now. The UK is still listed with 3 cases, too. (The actual number of cases is 8).

    Just as this epidemic (the WHO still insists it is not a pandemic as, according to (?their) guidelines, only 28/195 countries of this world affected cannot be classified as a pandemic ) was made public I went on flight tracker (it seemed the logic thing to do as it was by then known that 2019-nCoV is a novel virus)

    Bearing in mind that within Europe there are major airports which are hubs for flight changes to and from Asia, there was no country other than Greenland or the arctic/antarctic that could be be expected to stay virus free depending on number of infected travellers, of course.

    This morning a in the Wirral quarantined man was going to sign himself out and leave. He was talked into staying but this occurrence led the government to declaring coronavirus a ‘serious and imminent threat’ to public health, thus being able to ensure the people in quarantine cannot sign themselves out prematurely.

    It is getting increasingly harder for me to find non-politically motivated and non-conspiracy theory motivated, trustworthy information about 2019-nCoV. On feeding into Google “who tip of the iceberg” the first result was usable (channelnewsasia). This was followed by 3 leads to the definition of “tip of the iceberg”.

    Here my growing concern with respect to the internet censorship (I cannot think of a more appropriate term, apologies) we are currently observing. Should the WHO have the power to appeal for internet censorship? After all, conspiracy theories are nothing new, e.g. Chemtrails, flat earthers etc. etc. Naturally, there will be plenty of new ones springing out the ground with respect to the coronavirus.     An increasing number of conspiracy theories is also a good indicator of an increasing number of disgruntled people. Happy, content people just aren’t interested.

    Shouldn’t the WHO’s actions only address the growing spread of the coronavirus rather than trying to silence criticism?
    We all are aware that there is media censorship in China, that the number of infected people may/may not be wrong and that people are being punished for disobedience. The most prominent example recently is Dr. Li Wen Liang, who sadly died.

    I apologise in advance. It is high time for a rant. Fresh air clears the mind, even if it is an only a 4 mile walk in still quite extreme weather conditions.
    During our short walk I did remember seeing somewhere a petition to sign for sacking the WHO director Tedros Adhanom. On clicking on that link I saw that at least 200000 people worldwide had signed it. At that stage I left the page. To me this is a pointless exercise.

    I have a question: is the current media control by the WHO the answer to this? REALLY????

    What happened to the thing we all are proud of – free speech?
    Shouldn’t we be asking the WHO for total transparency first? Moneys incoming from where and how this is spent?
    I am extremely frustrated. 2019-nCoV is a novel virus that has crossed borders. Naturally, people like me just want to observe. Nothing more.
    It is easy to disregard conspiracy theorists and politically motivated people.
    It is tedious and time consuming to get past the WHO censored internet.
    [To me the WHO website is just a muddle and there are plenty of words there (lagging at least a day behind) saying very little.]

    I have disregarded the WHO as a source of information along with the conspiracy theorists from the beginning of this coronavirus epidemic (?pandemic). Ok, even before that, due to the – in my view idiotic – pursuit of becoming the world’s first life style police.

    During our short walk, taking a break on a bench with a bottle of beer (no no alcohol prohibition signs spotted) and a cigarette or two I did remember a comment from Dmitry in which he cynically mentioned all our finger pointing towards China when this coronavirus epidemic started.
    Right now I would love to hear/read what his take of this coronavirus epidemic is.

    Self isolation in our neck of the woods should not be a problem. Shopping for necessities online (including pharmaceuticals!), and signalling to the delivery driver through closed window/door to just drop everything right outside the door would be one suggestion. There are online places like skype/mobile phones etc. to keeping in contact with friends/family and TV/Netflix etc. etc. for entertainment, not to mention the internet to follow the ongoing epidemic.

    Operated by Princess Cruises, the Diamond Princess has 2,666 guests and 1,045 crew.

    It has been docked in Japan’s Yokohama port since February 4 after dozens of people became infected with the virus.

    So far at least 219 people (including five crew members) have tested positive for the coronavirus onboard after 44 more people were confirmed on Thursday.

    I am a little concerned that the company owning the Diamond Princess had not looked intensively into what quarantine measures include. 5 more staff were diagnosed.

    Apologies for just a brief update last night about the floating Petri-dish Diamond Princess. I was just too tired as I had spend the last few days in lock down, reading, reading, reading, reading.
    I am sure that the conservative web sites listed provide an interesting read and even better speculations but I’d rather stick to scientific material.

    For people interested:
    With respect to the Diamond Princess. The US has taken it’s citizen off the Diamond Princess and they are on their way home. I am not sure if this was a wise decision as Japan can hardly be classed as a poor third world country but I trust the US to have excellent quarantine facilities so further run-away trains should not get more ammunition.
    For now the rest of the passengers and crew have to stay put for yet another 14 days. Hopefully the remaining crew will be quarantined in cabins as well and a new crew (well cladded up) will provide for all in quarantine there.

    With respect to the MS Westerdam cruise ship which was refused docking in 4 Asian ports and finally allowed to do so by Cambodia eventually had 1 case diagnosed.

According to information from the German Press Agency, some are still on board the “Westerdam” in Sihanoukville, while others have already started their journey home. The guests on board the ship are to be tested for the virus.
    An infection was surprisingly found on the weekend of “Westerdam” passengers who had landed in Cambodia. An 83-year-old American was tested positive for the onward journey in Malaysia and taken to the hospital, according to the Malaysian health authority.

    According to the shipping company, 57 Germans were among the travelers. According to information from the German Press Agency, some are still on board the “Westerdam” in Sihanoukville, while others have already started their journey home. The guests on board the ship are to be tested for the virus.

    I am a little surprised, though, why smoking is such a priority (?WHO) in this outbreak.

    Draconic China’s measure may seem. However, the number of patients recovered vs the number of critical patients is getting very close now. Once the number of patients recovered exceeds the number of critical patients an end of this outbreak is in sight.

      The Department of Health and Social Care confirmed today that it has block-booked the hotel indefinitely, as part of measures to control the spread of coronavirus.

      Doctors will be sent to the site to carry out the tests, and anyone with a positive result will then be sent on to specialist hospitals for treatment.

      Officials hope it will limit the number of passengers travelling into London – and beyond – when they are infectious.

      The rooms – which normally cost £80 a night – will be used to isolate suspected cases while test results are awaited.

        I wish someone would tell Dr. Tedros that the Chinese already are transfusing blood plasma from blood donated by recovered patients who by now are beginning to produce an antibody to this virus. (Chris Martenson talked about this in yesterday’s video!! For some reason he hasn’t uploaded a video today, I was quite looking forward his next ).
        It seems the best way to go. The number of recovered patients is steadily rising and I take it, those who had the mild version of it are happy to donate blood.

    I know both by heart – elf’n’favety was at least once/year in our drill. I know all variations by heart. And it is a rather extensive drill,
    Isolation (what was done in the Wirral for the first negative patients) is easy. Yet I saw on videos the passengers of the Diamond princess unable to cope over a period of 10 days.
    Bad news for you guys. Quarantine is a lot worse.

    The Japanese professor Kentaro Iwata, an epidemiologist at the University Hospital of Kobe University, spent a day at the Diamond Princess as a volunteer. In multiple YouTube videos, he strongly criticized government policy.

    “The cruise ship was completely inadequate in terms of controlling infections,” he said in one of them.

    “There was not a single infection control professional on the ship, nor anyone professionally involved in infection prevention. The bureaucrats were in charge of everything.”

    “There is no clear distinction between the green (healthy) zones and the red (potentially infected) zones. And the staff is running back and forth,” Iwata reported.

    the Ministry of Health reported that 79 further cases of infection had occurred on board. A total of 621 people are now infected, most of whom have been taken to hospitals.
